RESUMO

Edentulism can generate negative impacts on self-esteem, interpersonal relationships, and oral functions. Removable prostheses are commonly used for tooth replacement, but they may cause discomfort due to micromovements during mastication. Objective and quantifiable methods are needed to evaluate these micromovements. A pilot study was conducted to determine the micromovements in removable prostheses during mastication using a 3D electromagnetic articulography (EMA-3D) system. One elderly participant wearing lower removable prostheses and an upper total well-fitting removable prosthesis was studied. The EMA-3D system was used to record movements in three spatial planes. Peanuts were given as test food, and the participant was instructed to chew normally while recordings were carried out until feeling the need to swallow. Analyses of the upper total prosthesis show micromovements ranging from 0.63 ± 0.11 to 1.02 ± 0.13 mm. During simultaneous analyses of the upper prosthesis and lower partial prosthesis, interference was not observed. This pilot study demonstrated the feasibility of using the EMA-3D system to evaluate micromovements in removable prostheses during mastication. Further research is needed to evaluate a larger sample and assess the clinical implications of these micromovements.

SUMMARY: The terms fossa and fovea are found in Terminologia Anatomica, however, there is no clear difference between both terms. Therefore, the objective of this study was to determine the coherence between the lexical, morphological and etymological relationship of the term's fossa and fovea with their use in Terminologia Anatomica. The terms fossa and fovea were consulted in Terminologia Anatomica (Federative International Programme for Anatomical Terminology, 2019) and in International Anatomical Terminology (Federative Committee on Anatomical Terminology, 2001). Subsequently, a search for both terms was carried out in Latin-Spanish and English-Latin dictionaries. Finally, the terms fossa and fovea were consulted in the Diccionario de la Real Academia Española and in the Diccionario panhispánico de términos médicos de la Real Academia Nacional de Medicina de España to know the definitions in the medical area. The search reported that the term "fossa" comes from the Latin fossa, which means "excavation, ditch, ditch; hole, hole." For its part, "fovea" comes from the Latin fovea, which means "small hole." Therefore, the difference between fossa and fovea is determined by their size according to their definitions. However, in Terminologia Anatomica, structures have been described with the diminutive of fossa and fovea, which makes the differentiation of the terms according to their size more complex. Furthermore, the lack of unification in the nomenclature between fossa, fovea and their diminutives generates great confusion and inconsistencies in the translations from Latin to English and Spanish in Terminologia Anatomica that reduce the precision of the nomenclature. Therefore, we propose the unification of the terms fossa and fovea based on the size of the anatomical depression and clarify their translation into English and Spanish.

RESUMO

Chewing is a complex procedure that involves sensory feedback and motor impulses controlled by the trigeminal system in the brainstem. The analysis of mandibular movement is a first approximation to understanding these mechanisms. Several recording methods have been tested to achieve this. Video, ultrasound, the use of external markers and kinesiographs are examples of recording systems used in research. Electromagnetic articulography is an alternative method to those previously mentioned. It consists of the use of electromagnetic fields and receiver coils. The receiver coils are placed on the points of interest and the 3D coordinates of movement are saved in binary files. In the Oral Physiology Laboratory of the Dental Sciences Research Center (Centro de Investigación en Ciencias Odontológicas-CICO), in the Faculty of Dentistry at the Universidad de La Frontera (Temuco, Chile) several research studies have been carried out using the AG501 3D EMA articulograph (Carstens Medizinelektronik, Lenglern, Germany). With this device, they developed a series of protocols to record mandibular movement and obtain new information, such as the 3D Posselt polygon, the area of each polygon, individualized masticatory cycles and speed and acceleration profiles. Other investigations have analyzed these parameters, but separately. The AG501 allows for holistic analysis of all these data without altering natural movement. A limitation of this technology is the interference generated by its metallic elements. The aim of the present work is to show the developed methods used to record mandibular movement in the CICO, using the AG501 and compare them with others used in several research studies.

RESUMO

Mandibular movement recording is relevant for the planning and evaluation of mandibular function. These movements can include mandibular border movements (MBM) or mastication. Our objective was to characterize the kinematics of MBM and mastication among skeletal classes I, II, and III in the three spatial planes. A descriptive cross-sectional study was conducted with 30 participants. Instructions were provided on how to form Posselt's envelope and to perform masticatory. After data processing, we obtained numerical values for the areas, trajectories, and ranges of MBM that formed Posselt's envelope and the values for speed, masticatory frequency, and the areas of each masticatory cycle. Significant differences were found in the area of Posselt's envelope in the horizontal plane between skeletal classes I and III and in the range of right laterality between skeletal classes II and III. Mastication showed significant differences in the area of the masticatory cycles in the horizontal plane between classes I and III and between classes II and III. In conclusion, there were differences in MBM and mastication between skeletal classes III and I in the horizontal plane. This study supports the need to establish normal values for mandibular kinematics in skeletal class III.

RESUMO

Temporomandibular disorders (TMDs) are a group of pathologies that affect the temporomandibular joint and its related structures, producing intracapsular and muscular pathologies. The aim of this study is to describe, by electromagnetic articulography (EMA) and simultaneous electromyography (sEMG), the mandibular postural position and mouth opening in healthy patients and with articular and/or muscular pathology. MATERIALS AND METHODS: A pilot study was conducted with a sample of sixteen participants aged 18 years or older who attended the TMDs and Orofacial Pain Polyclinic of the University of La Frontera due to TMDs. The physiological inoculation space was evaluated from the mandibular postural position (MPP) with swallowing command and without command, in both healthy patients and patients with articular, muscular, and mixed TMDs, measured simultaneously with EMA and sEMG. An angular measurement of the oral opening was also performed with the data obtained. RESULTS: The physiological inoculation space was obtained from the determination of the MPP through the procedures with swallowing command and without command, and different mouth opening degrees were evaluated. CONCLUSIONS: Simultaneous position and sEMG records can be produced from EMA, and different characterization variables such as the vertical distance, Euclidean distance, and angle can be obtained.

RESUMO

Bone loss is a common problem that ranges from small defects to large defects after trauma, surgery, or congenital malformations. The oral cavity is a rich source of mesenchymal stromal cells (MSCs). Researchers have documented their isolation and studied their osteogenic potential. Therefore, the objective of this review was to analyze and compare the potential of MSCs from the oral cavity for use in bone regeneration. METHODS: A scoping review was carried out following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses extension for Scoping Reviews (PRISMA-ScR) guidelines. The databases reviewed were PubMed, SCOPUS, Scientific Electronic Library Online (SciELO), and Web of Science. Studies using stem cells from the oral cavity to promote bone regeneration were included. RESULTS: A total of 726 studies were found, of which 27 were selected. The MSCs used to repair bone defects were (I) dental pulp stem cells of permanent teeth, (II) stem cells derived from inflamed dental pulp, (III) stem cells from exfoliated deciduous teeth, (IV) periodontal ligament stem cells, (V) cultured autogenous periosteal cells, (VI) buccal fat pad-derived cells, and (VII) autologous bone-derived mesenchymal stem cells. Stem cells associate with scaffolds to facilitate insertion into the bone defect and to enhance bone regeneration. The biological risk and morbidity of the MSC-grafted site were minimal. Successful bone formation after MSC grafting has been shown for small defects with stem cells from the periodontal ligament and dental pulp as well as larger defects with stem cells from the periosteum, bone, and buccal fat pad. CONCLUSIONS: Stem cells of maxillofacial origin are a promising alternative to treat small and large craniofacial bone defects; however, an additional scaffold complement is required for stem cell delivery.

Fibro-osseous lesions are considered benign and make up a group of disorder pathologies that are characterized by the replacement of normal bone by tissue composed of collagen fibers, fibroblasts, and mineralized tissue. We present a radiographic finding obtained from a 41-year-old male patient who attended a radiology center for a panoramic radiograph. The examination revealed an interesting radiographic finding in the left hemi-mandible, where a fibro-osseous lesion was observed. with bone expansion at the level of the marginal ridge and basal mandibular cortex, displacement of the mandibular canal, dental displacement, compromise of the alveolar cortex and rizalysis at different levels in the teeth adjacent to the lesion. According to the previous antecedents, a long-standing diagnostic hypothesis of Ossifying Fibroma is established due to its radiopacity. The World Health Organization classifies it as a benign bone neoplasm affecting the craniofacial skeleton, with the highest incidence in the jaw, generally presenting between the 3rd and 4th decade of life. We conclude that fibro-osseous lesions can be detected as a radiographic finding, this is relevant for early treatment, however the diagnosis must be made by complementing the clinical and histopathological history of the lesion, paying special attention to the differential diagnosis.

The search for an ideal occlusion is one of the main objectives of rehabilitation treatments. For this to be fulfilled, some characteristics need to be present, based on this, the concept of "centric triad" has been constituted, which is defined as the coincidence between the physiological centric relation (CR), muscular contact position (MCP) and maximum intercuspal position (MIP), thus existing an optimal morphofunctional balance of the different components of the stomatognathic system. In this way, the need arises to analyze the concepts that involve the relationship between the different mandibular positions, within these concepts we find the "centric area", which due to its characteristics, it is attributed relevance and clinical utility when its application is understood. The objective of this study was to analyze the concept of centric area, update the associated occlusal concepts and propose new methodologies for its evaluation. Sometimes there are occlusal interferences that prevent the jaw from continuing its usual path to reach a certain position, these interferences are known as premature contacts, when they exist, the jaw naturally deviates towards a position of accommodation. Centric slide is defined as the movement of the mandible when it is in a retruded contact position unforced, that is, the contact position that occurs when the mandible closes in CR, until reaching MIP, this sliding occurs due to in the presence of premature contacts, when there are no premature contacts, this position will coincide with PMI. The centric area is defined as the slippage of the mandible between the retruded ligamentous position (RLP) and MIP. Ideally, the PMI occurs when the condyles are in CR and coincides with the MCP, there should always be a centric area, since physiologically no joint will naturally adopt an extreme position like PRL.

The pressure to publish today has led many researchers to commit scientific misconduct, fraud being the most serious of all. This occurs in the form of fabrication, falsification, plagiarism, authorship problems, image manipulation, and redundant posting. Scientific fraud is defined as deliberate misrepresentation by someone who knows the truth. In the history of humanity, important cases of scientific fraud have been known, among them the following can be highlighted: the Piltdown man, the Shinichi Fujimura case, the vaccine scandal, the Pearce case, the Yoshitaka Fujii case, among others. In order to neutralize fraud, different strategies have been developed to detect it, among them we find: evaluation by peer reviewers, Responsible Research Conduct (RCR) programs, regulations that the scientific community itself carries out, where we find the PubPeer Foundation and the For Better Science blog. Similarly, different measures have been imposed to counteract fraud, such as: transparency of pressures and opportunities, public availability of the data that support the hypothesis, and public denunciation of scientific fraud. The impact of a fraud has important consequences for science, studying from false or wrong information leads to a great setback in scientific advances in the world today. It is the responsibility of each one to be aware of what is written and what is read, since as is known, that is the only way to combat it. As researchers we are responsible for acting ethically in our research and being aware of the measures that exist today to detect and combat scientific fraud.

SUMMARY: The terms disc and meniscus are found in Terminologia Anatomica. However, both terms are used synonymously to refer to the intra-articular cartilage present in the temporomandibular joint (TMJ). For this reason, this study aims to expose the greek roots of the terms disc and meniscus to evaluate the coherence between their lexical and morphological relationships. Both terms were consulted in the International Federative Program of Anatomical Terminology and in Greek, Spanish and, English dictionaries. The search reported that the term "disc" derives from the Latin discus, and this in turn from the Greek δίσκος [dískos], which means "cylindrical body whose base is very large compared to its height". For its part, "meniscus" comes from the Greek mēnískos μηνίσκος [mḗn μήν gr. μείς 'meis', 'crescent moon' + -iskos Gr. Diminutive suffix 'small'] which means "small crescent moon" and, is defined as "cartilage with a semilunar shape and decreasing thickness from the periphery to the center; It is part of the knee joint. Therefore, the anatomical definition of articular disc and meniscus is determined by its morphology according to the etymology of the word. In the TMJ, an articular disc is found between the articular bones, depending on their morphology.

SUMMARY: The objective of recording mandibular movements is to obtain relevant parameters that allow the evaluation of the state of the temporomandibular joint (TMJ) and of the muscles involved in mastication. Mandibular movement is due to a complex set of three-dimensional rotations and translations performed by the TMJ, limited by the ligaments and their articular surfaces, and by the morphology and alignment of the teeth, when the mandible moves over these limits, they are called mandibular border movements. The aim of this article is to provide an updated description of mandibular movements through electromagnetic articulography. Mandibular border movements are classified according to the plane of space in which they move, thus we have border movements in the sagittal plane, in the horizontal plane and the frontal plane, and in each plane they draw a different polygon, called Posselt polygons. These polygons can be recorded by electromagnetic articulography, thanks to this technology some interesting parameters can also be extracted, such as: the total trajectory covered by the mandible when describing each polygon, displacement ranges in each plane, total area of each polygon of Posselt. Mandibular opening is measured as the distance between the incisal edges of the upper and lower central incisors at maximum opening plus the overbite. The analysis of mandibular opening is an important part of the clinical examination in dentistry as a preliminary evaluation, since limited opening can be the result of temporomandibular joint disorders, submucous oral fibrosis, rheumatic disease or facial trauma. The three-dimensional measurements that are obtained thanks to the electromagnetic Articulography, are appropriate to determine the normal ranges of opening, its accuracy allows the realization of investigations in the area, opening a wide field in the analysis of mandibular movements. RESUMO

Considering current water situation, reuse is an effective solution to meet water demand and reduce pressure on conventional water sources. However, pharmaceutical and personal care products (PPCPs) in effluents from wastewater treatment plants (WWTPs) decrease their quality and suitability. With the aim of identifying and monitoring both the influence of PPCPs and the suitability of effluents to be reused, this study proposes the development of a composite indicator (CI) related to PPCP presence in WWTPs, through the common weight multi-criteria decision analysis (MCDA)-data envelopment analysis (DEA) model. Obtaining a CI for PPCPs is a novel approach in the published literature, showing a new perspective in PPCP management and their influence in wastewater treatment. Furthermore, this study proposes an improvement on MCDA-DEA model which maintains the initial hierarchy obtained for the units analyzed. The development of CI is based on information about the technological, environmental, social, and biological issues of WWTPs. Results show that 4 of the 33 WWTPs analysed had the best CI values, meaning that their effluents have lower environmental impact. The development of a CI related to PPCPs in WWTPs suggests that further steps are needed to manage the WWTP effluents. Hence, the need to implement preventive measures in WWTPs has been shown, even though the removal of PPCPs is not yet part of European law. This work highlights the importance of considering PPCPs as priority pollutants in wastewater management and reuse frameworks, to guarantee low environmental impact and adapt wastewater reuse based on a circular economy approach. HIGHLIGHTS: Emerging contaminants (PPCPs) are used as effluent quality indicators. A composite indicator for PPCPs performance has been developed through MCDA-DEA model. Indicator obtained allow decision makers implementing concrete actions to assess effluent quality. Results show the improvement capacity of the effluents quality through PPCPs removing.

RESUMO

PURPOSE: To determine the effectiveness of palatal brushing in the treatment of denture-related erythematous stomatitis (DES) in complete denture wearers. METHODS: This two-parallel-arm RCT was conducted in three university clinics in Brazil, Canada, and Chile. Participants (n=77) were randomly allocated to receive (i) instructions for palatal brushing and standard oral/denture hygiene ("intervention"); or (ii) standard oral/denture hygiene instructions only ("control"). Data collection was carried out at the baseline and at 3 and 6 months after intervention. Outcomes included the magnitude of oral Candida carriage and the degree of inflammation of denture-bearing tissues. Groups were compared using generalized estimating equations and chi-square test (α=0.05). RESULTS: Palatal inflammation levels were reduced significantly in the "intervention" compared to "control" group at 6 months (intervention: 70%, control: 40%; chi-square, p=0.04). There was no between-group significant difference in the Candida count from denture and palatal biofilms; however, a subgroup analysis restricted to baseline Candida carriers showed further reduction with the intervention at 6 months. No adversity was observed by trialist or reported by participants. CONCLUSIONS: Including palatal brushing in oral instructions for denture wearers has positive impact on DES-related mucosal inflammation. Thus, our findings endorse the inclusion of palatal brushing in standard oral hygiene instructions to treat DES.

SUMMARY: The rat jaw is one of the most interesting anatomical structures in dental science research, due to its similarity to the human jaw. The aim of this study was to morphometrically characterize the mandible of rats by CBCT. The cone beam examination of 20 healthy adult female rats was analyzed. Using the examination software, it was possible to isolate the right and left mandible from the other structures and adequately perform the morphometric measurements of interest. In the comparison of both sides, only two measures presented significant differences. The distance from the deepest point of the sigmoid notch to the deepest point of the notch at the lower margin of the mandible was greater on the left side than on the right. And the distance between the most superior point of the condyle to the mandibular base, in the perpendicular projection of the mandibular plane, was greater on the left side than on the right. According to the measurements obtained, we can determine that the rat jaw is an elongated structure, with larger dimensions in the anteroposterior direction compared to the vertical measurements. Knowledge of the anatomy of the rat mandible and its variations per side allows researchers to have adequate references for studies involving this structure, such as research that requires the application of anesthetic techniques, study of orofacial pain, pathophysiological study of the TMJ, study of bone growth, testing of dental biomaterials or in the study of pathophysiological processes related to den- tal or periodontal tissues. The rat mandible is a good animal model due to its reproducibility and low cost. The right and left mandible are similar in their anteroposterior dimensions, but not the vertical ones.

RESUMO

Purpose: Obesity has become a growing public health issue worldwide. Studies have shown that eating rate is one of the most important factors to consider in the strategies to prevent and/or treat obesity. Eating rate can be reduced through different strategies, such as an increase in oro-sensory exposure, the modification of food texture, and an increase in the number of chewing cycles. The aim of this systematic review was to analyze the available evidence regarding the effect of chewing behavior modification on the parameters that contribute to obesity. Methods: A systematic search was done on the electronic databases Pubmed, Cochrane Central Register of Controlled Trials, and Scopus, using the terms "mastication", "chewing", "chewing speed", "prolonged chewing", "number of chews", "masticatory cycles" "satiety" "satiety response" "appetite", "appetite regulation", "nutritional status" and "obesity". Results: A total of 23 intervention studies were selected that intervened in the participants' chewing behavior, either by reducing the eating rate, increasing oro-sensory exposure, food hardness or the number of chewing cycles. In most studies these interventions were effective at reducing food intake, subjective appetite and improving the plasma levels of satiety-related hormones and metabolites; moreover, they reduced body mass index in the long term. Conclusion: The currently available evidence seems to indicate that modifications to chewing behavior can bring with it a myriad of benefits for the treatment of obesity.

The mouth opening is an important indication of the functionality of the temporomandibular joint (TMJ). Mouth opening is usually evaluated by asking the patient to open their mouth as wide as possible and measuring the distance between the edges of the frontal incisors with a ruler or caliper. With the advancement of technology, new techniques have been proposed to record mandibular movement. The aim of this work is to present a novel technique based on 3D electromagnetic articulography and data postprocessing to analyze the mouth opening considering distances, trajectories, and angles. A maxilla-mandible phantom was used to simulate the mouth opening movement and fixed position mouth opening. This was recorded using the AG501 3D EMA (Carstens Medizinelektronik GmbH, Bovenden, Germany). The collected data was processed using Matlab (Mathworks, Natick, MA, USA). Fix and mobile mouth opening of 1, 2, 3 and 4 cm were simulated. It was possible to evaluate the mandibular opening through the vertical distance, the Euclidean distance, the trajectory, and the opening angle. All these values were calculated and the results were consistent with expectations. The trajectory was the highest value obtained while the vertical distance was the lowest. The angle increased as the mouth opening increased. This new technique opens up new possibilities in future research since oral opening can be analyzed using multiple variables without the need to use different devices or depending on the researcher's experience. This will make it possible to establish which parameter presents significant differences between groups of patients or between patients who have undergone some treatment.

RESUMO

Osseointegration implies the coexistence of a biocompatible implant subjected to masticatory loads and living bone tissue adhered to its surface; this interaction is a critical process for the success of implants. The objective of this work is to analyze the osseoformation and osseointegration of a dental implant in operation for 5 years microscopically through morphological analysis of the surface and chemical composition through a variable pressure scanning electron microscope (VP-SEM) and energy dispersive X-ray spectrometry (EDX). The chemical composition and general characteristics of the structural morphology of random areas of the surfaces of an osseointegrated dental implant from an ex vivo sample were analyzed. On the surface of the implant free of bone tissue, titanium (TI) was mainly identified in the area of the implant threads and carbon (C) in the depth of the implant threads. Phosphorus (P), calcium (Ca), oxygen (O), carbon (C), with dense and homogeneous distribution, and, to a lesser extent, sodium (Na) were detected on the bone surface around the contour of the implant. Regarding the morphological characteristics of the implant surface, a rough structure with some irregularities and detachments of the implant lodged in the bone tissue was observed. Microscopic analysis showed calcified bone tissue distributed in an orderly manner on the coronal and medial surface and sinuous and irregular in the apical area, with the presence of red blood cells. The composition of the implant allows a dynamic process of bone remodeling and regeneration subject to the biological and mechanical needs of the operation. Dental implants are shown to have exceptional and long-lasting biocompatibility that enables the formation of mature peri-implant bone tissue.

RESUMO

Mesiodens are the most common supernumerary teeth and are detected incidentally during routine radiographic examination, so late diagnosis complications are very common. The dentist must make a timely diagnosis and thus avoid clinical complications. Despite advances in knowledge of dental morphogenesis and differentiation, the etiology of mesiodens remains unclear. Therefore, several theories have been postulated to explain how and why they develop. It was described in the literature that heredity could play an important role in the appearance of supernumerary teeth, with a higher rate of appearance in relatives of those affected. This article reports three cases, a mother and two children, who present mesiodens, which shows that supernumerary teeth may involve a genetic factor. In addition, a literature review was carried out to assess the importance of the genetic factor as a possible cause of mesiodens. The relevance and implications of timely diagnosis in clinical practice to avoid manifestations of clinical complications are discussed. Therefore, the identification of the genetic risk factors responsible for the formation of supernumerary teeth is essential for developing a screening tool to determine an individual's genetic risk.
